It depends how much of a hassle you want to go to, how much volume you need,  and how important the "look" is to you. In my case, I really like the 
way the stock radio "looks" so I retain it. I've found (especially in my coupe with  a loud exhaust or my vert with lots of wind noise, that a louder system 
is important.

For both cars, I use a $20 black plastic box to mount the aftermarket stereo under the dash. With that, you're not limited to any certain kind of system.

For a late model vert, you have to get a little creative.  Here's what I did for under $500:

JVC Head unit with cassette. 4x40w, under $100
JVC 12 disc changer on shelf in trunk (direct control by head unit) $100
Pair of 6" Sony Explode 3 way speakers mounted in place of the stock speaker on a homemade frame. About $35
Pair of cheap piezo supertweeters at each a-pillar to help with stereo separation $20
3 channel 150w/ch amp under passenger seat $35 (Walmart-clearance)
Driven by the amp, two 6x9 two way speakers (Pioneer, $35) mounted in junk side panels (Ebay $35) behind vintage-looking chrome grills (Ebay $20)
And finally two Bass Shakers Pro ($75) driven by the third channel. One under the rear seat, one under the driver's seat.

Modifcations to the car: two 3/32 holes for each bass shaker and the amp.

Looks fairly "period" but moreover unobtrusive. The face plate is removable for security and once it's off, you hardly even notice the underdash mount.

Sound is LOUD (was just out driving the car at lunch) and moderately clean (given the price and the desire not to cut on the car)

In my coupe, I did not use the extra amp, have no yet added a changer, and used a powered subwoofer and two box speakers on the rear deck.
