Similar on my station wagon. The plastic roller wears out and is then
small enough to jump out of the rail.
Got a replacement roller, two halves (?), but do not like the suggested
big procedure. Sometime in future I will try to drill the rivet and
make a custom attachment of a custom plastic roller. Or so ...

Meanwhile the recent TUEV / tech inspection report had two issues, one
of them:
 - Window crank driver's side missing
