Rick is correct -- btdt (but in a street car).  It has usually happened
to me when my clutch disk got worn and thin.

It is possible to overcenter the diaphragm spring fingers on the
pressure 
plate. I have experienced it in my race car during a fast gear change.
We 
usually have a adjustable pedal stop to prevent it.
