Are you sure that it was not the shaft as well as the bearing riding up 
with it?    If so, that is normally caused by pressing the bearing shaft 
out without properly heating the shaft and mating housing.  If you just 
press the shaft out cold, then the hole mating to the bearing shaft 
becomes broached and can fail once the engine becomes hot.   I had a 
failure like that happen with an idler shaft that came out when the 
engine got hot.

The fan wanted to rise up while driving (I am glad I had the top shroud on so as to contain the fan, pulley, bolts, and bearing). I actually had to hammer it back down in a few places on the side of the road but it was in Williams that the belt came off as well.
