Has the problem developed around the time of a valve adjustment? Severe
misadjustment of the valves can cause this. No harm in adjusting them again
(they don't get out of adjustment on their own) if you've exhausted (excuse
the pun) other alternatives.
     You can do it just on the side in question, of course. And yes, hot OR
cold.
     Severe valve LEAKS can do this, too. Do a compression test.
