Does anyone have a diagram indicating the proper location of rear seat belts for 
a '64 convertible to share?

Rear seatbelts are available from Clark's and Corvair Underground, but I prefer the reinforcement hardware from Andover Restraints (http://www.andoauto.com/).  Check them out if you haven't already purchased your seatbelts.
