<VV> found cause of damaged piston, and is there a dipstick tube o-ring?

Our theory is that at some point there was excess lash on this valve
allowing the pushrod to dance around, at some point the block end of the
pushrod came down on the retainer clip instead of in the dimple.. causing
the valve to hit the piston and breaking or knocking loose the retainer clip
in the lifter.
