Andy,  The common explanation that I hear is that the ring grooves 
temporarily lined up with one another to allow oil to pass to the top of the 
piston.  I've also seen that happen (the smoke cloud) when the car was 
parked on an uneven surface - so that one bank of cylinders was on the 
downhill side.
