Hi Terry,
It's been so long since i worked with these cams, i just can't remember  some 
stuff. For example, CWC is the manufacturers ID but i can't remember the  
actual name. So that would place it as a stock and probably a replacement  cam.
 
The rest of the numbers don't ring any bells, but at 0.257 lift, it would  
most likely be a 889 cam. A 891 cam would check out at 0.260 in.
 
So the evidence would definitely point to a LM 95 hp cam.
