it is possible to do it without pulling cylinder off the piston.(LATE 164  
cu inch) ..put that piston at top of stroke , pull cylinder until oil ring 
just  shows in the slot in bottom of cyl, then there is just enough room to 
get my air  powered drill in there with the helicoil drill. i modified the 
helicoil plastic  driver by grinding the ears off it.  i have done several of 
these over the  years with no problem.
if you have a good "take apart" ring compressor , and the time, i  would 
remove the cylinder , poke a carefully arranged rag in the hole around the  
rod , and run a good shop vac while drilling and tapping. then remove the rag 
,  and put it back together. i used a small longnose pliers  i magnetized  , 
to poke the tip off the helicoil, 
hope this helps. 
one block (WB) i did was ready to be tossed because of a broken tap in a  
top stud hole . i ended up blowing the tap out with a torch. it came out so  
clean , i hardly had to run a tap thru the hole to clean it. you just have 
to  get brave for some of these situations , and "deal with it" ?
 somewhere i have pictures of how I do it to post...
my suggestion:  try a helicoil tap in that hole , if it is snug go for  it!
