Now that the fuel nozzles mostly all have a plastic coating or "hood" on 
them, turning the nozzle upside down and resting it against the inside 
of the fuel door is much easier than just holding it upside down, like 
we used to have to do to preserve the paint on the inside of the door.  
Back when people were actually paid to pump gas (those are still jobs in 
Oregon), it didn't take long to learn how to fill various vehicles -- 
the worst were the driver's side tank on some early seventies 
International Travel-Alls (a situation that could be repaired by 
altering the factory installation of the hose to the tank -- another 
brand's case of the factory isn't always right).

Also depends on the flow rate of the pump one is using -- if it is slow 
enough of a pump, one can get away with the "normal" position.  Few 
pumps are that slow anymore, even on the slowest setting.
