<VV> AC/Heater Fan Switch

I have a 66 Factory Air Conditioned (110 HP engine) that needs a Fan Switch.  The 
switch mounts at the top of the 4 vertical sliders and has 4 positions, off, low, medium 
and high.  Any help locating
