You can get adapters for the different bolt patterns from a number of 
places (e.g. http://www.skulte.com/adapters.html).  Getting the wheels to 
fit within the fenderwells may be more difficult.

When doing a disc brake conversion you need bigger wheels to accomodate the 
rotor/caliper combination you've selected.  The wheels should also be open 
to help with rotor cooling.  You're somewhat limited on front rotors 
because they must have a small "hat" (otherwise they'll hit the large 1/2" 
spindle bolt).  A good candidate for front rotors are the rear rotors from 
a 79-81 Trans Am.  They have an 11-1/8" diameter and will fit most 15" and 
larger wheels.  You might want to wait on wheel selection until you've 
selected your rotors/calipers.
