Just went thru this on the CC Forum....

Are you certain that NUT is   holding?   The nuts are "Ovaled" to make then 
"self locking"

They wear out  over time [ adjustments]  and  will back off quickly!

With a torque wrench..they should take approx. 20 ft lb.  to  turn
