There is the stipulation that such tags will cost the driver 
half-a-grand.  The money is supposed to go (so it was supposedly 
originally supposed to have been) into a slush fund to pay for 
anything that driver might run into.   However:

I'm told that it's not exactly the case and that if it does happen, 
and you paid that uninsured motorist fee, the commonwealth will not 
actually "cover" you.  That leaves you a target for a whopping 
lawsuit that you are unlikely to win and that the commonwealth will 
be very unlikely to provide any help for anything.  In fact, they're 
likely to come after you as well for everything from paperwork 
expenses to cleaning up the pavement if your blood drips on it from 
that cut over your eye when the guy you ran into punches you.  Your 
ass will be at the mercy of the courts and the other driver's 
insurance company which WILL take you to the cleaners.

This, from somebody at the DMV here in town.

Go cheap and take your chances, or buy real insurance.
