I am another old time computer guy. Vintage 1965 IBM 7080. Life insurance application. Really bad news if it doesn’t work correctly and on time. The key was pre-installation testing. A test case for every decision point in the system. 
Fast forward to today and the test function is performed by the user out here in the real world. No wonder so many devices don’t work, or don’t have operating directions, etc.
