When I was still at the dealership we would have a stack of defects, maybe 
upwards of 20, go back every month or so.  My guess is 90% would have the 
terminal pulled out or cracked. The service manager once told me that the batteries 
were not held down properly OR were over-tightened.... tighten it just enough 
so you can't move the cable, and make sure the hold down is attached 
properly.... (and YES, some were from BRAND NEW CARS off the truck, so I'm not pointing 
fingers at the shops or at the owners).
I hope this helps.... just chalk it up to "human error".
